The deadline for registration and hotel accommodations is April 25, 2008.  We prefer that you register online.  However, please feel free to download the printable registration form to submit by email or print and fax/mail it in as well.

Printable Registration Form

• You can submit the printable registration form in one of three ways, by email (preferred), by fax, or by US mail.
• You will need to have Adobe Reader to open the Registration form.
• If you do not have Adobe Reader, please download it: http://www.pdf-suite.com/step1.asp

To Submit by Email

• Click the “Submit by Email” button.
• If you use an email application such as Microsoft Outlook Express, Microsoft Outlook, Eudora, or Mail, make sure “Desktop Email Application” is selected and click “OK”.
• Click “Send Data File”.
• Your email client’s message should appear.  Click “Send”.
• If you use an email application such as Yahoo or Microsoft Hotmail, select “Internet Mail” and then change "OK".
• Click “Save Data File” and choose where you would like to save your registration form.
• Open up your email application, and compose a new email message to warren1@recsports.wisc.edu.  List “Big Ten Registration” as the Subject.  Attach the data file you saved, and click “Send”.

To Submit by Mail

• Click the “Print Form” button, and click “OK” to print.
• Complete the printed Registration form.
• Mail the completed form to:
